Identical vs. Fraternal: What the Difference Actually Means

The distinction comes down to conception. Identical twins, known medically as monozygotic twins, occur when a single sperm fertilizes a single egg and the resulting embryo splits into two. Because they originate from the same fertilized egg, they share the exact same DNA. Fraternal twins, or dizygotic twins, develop from two separate eggs fertilized by two separate sperm. They share DNA in roughly the same proportion as typical siblings and can be different sexes.

In extremely rare circumstances, fraternal twins can even have two different biological fathers. Because their genetic profiles are distinct, DNA testing is the only reliable way to tell fraternal twins apart at the molecular level.

Why Looks & Birth Records Aren’t Enough

Appearance can be misleading, especially when twins are young. Many fraternal twins look strikingly similar, and some identical twins develop enough physical differences over time that families genuinely aren’t sure. Looking at them isn’t a reliable test.

Birth records aren’t foolproof either. Doctors have traditionally classified twins at birth by observing the placenta, assuming one shared placenta means identical and two separate placentas mean fraternal. That’s not always accurate. An egg that splits early in development can still produce two separate placentas and two separate amniotic sacs, meaning identical twins can be misclassified as fraternal from birth. The only way to know for certain is a twin zygosity DNA test.
